Visit Information


Please make an appointment for every office visit; however, if you have an urgent need, it is likely we can accommodate you with an appointment the same day.
 

  • We offer same-day appointments for established patients.

  • We will make every attempt to schedule same day visits for new patients for simple medical problems, but on the same day visits, we will not provide routine maintenance healthcare. We will only take care of the immediate problem and schedule a new appointment for routine care.


If you are a new patient, please arrive at least 15-20 minutes before your scheduled appointment, so we may verify your insurance and ask you to complete your first visit forms.

When coming to your first appointment, please bring with you:
 

  • Medication list

  • Past medical history

  • Insurance card

  • Prior medical records, if available

  • Your immunization record

  • Co-payment

  • Parents, please bring your children’s immunization records.


If you are a new patient, please complete this New Patient Registration Form and bring it with you to your first visit.



For all other appointments, please bring with you each time:
 

  • Photo ID

  • Current insurance card(s)

  • Co-payment(s), if required

  • Tests results/films that have been requested by us

  • If you have Medicare, please bring your pharmacy insurance drug card.

 

We strive to keep your wait time to a minimum. To help us best serve all patients, please be on time for your scheduled appointment.

Pay Your Bill Online
